Your Checkup Checklist There's a lot of information to cover during a doctor's visit, making it easy to become overwhelmed. Before your next appointment, take a few minutes to prepare. TexasHealth.org 4 Welcome, New Physicians! • Create a list of medications you're currently taking, including prescription and over-the-counter medications, vitamins and herbal supplements, to share with your provider. • Write down any questions or concerns you have about your health in a small notebook and bring it with you. You can also use the notebook to write down important details or notes from your conversation for later reference. • Remember to bring your health insurance card and photo identification. Here's a closer look at the newest additions to the medical staff. MAURICE ALAZAR, M.D., INTERNAL MEDICINE PHYSICIAN Dr. Alazar obtained his medical degree from Seton Hall University. He is certified by the American Board of Internal Medicine and the Syrian Board of Internal Medicine. Dr. Alazar comes from MidMichigan Medical Center, where he served in a variety of capacities, including as a hospitalist and internal medicine physician and overseeing the hospital medicine program. MOUSTAFA BANNA, M.D., CARDIOLOGIST Dr. Banna graduated magna cum laude with a Bachelor of Arts degree in economics from Case Western Reserve University. He then received his medical degree from the University of Cincinnati College of Medicine. He completed an internal medicine residency at the University of Pittsburgh before completing a clinical electrophysiology research fellowship at the Cleveland Clinic Foundation. Audrey Puentes, M.d., OBSTETRICIAN AND GYNECOLOGIST Dr. Puentes received her Bachelor of Science degree from The University of Texas at Austin before obtaining her medical degree from The University of Texas Medical Branch. She is a member of the American Medical Association, Texas Association of Obstetricians and Gynecologists and American Congress of Obstetricians and Gynecologists. FAIZ A. MIRZA, M.D., GASTROENTEROLOGIST Dr. Mirza graduated from Northwestern University with a Bachelor of Arts degree in psychology. He then earned his medical degree at the Feinberg School of Medicine at Northwestern University before completing an internal medicine internship and residency at McGaw Medical Center of Northwestern University.
