This commitment to innovation also includes programs that promote a smooth transition back to home and work life." — Gladwyn Baptist, M.D., Cardiologist Heart Programs Don't Miss a Beat The cardiac team at inspira Health network has implemented several innovative programs to improve care for patients with heart disease. • Care Transitions — This Inspira Medical Center Woodbury program works with patients at risk for hospital readmission to coordinate follow-up care and other assistance as needed. • Collaborating Options Across the Continuum of Healthcare (COACH) — Through the Inspira Medical Center Vineland's COACH program, a case manager consults with patients at the bedside and in the home post-discharge to ensure a smooth transition of care following a heart attack or other condition that increases readmission risk. • Community Paramedicine Program — In this pilot program launched in May 2013 by the Care Transitions team at Inspira Medical Center Woodbury, a paramedic visits a patient's home to assess needs, review and reconcile medications, conduct a safety inspection, and confirm follow-up appointments. • Center for Heart Failure — Since late 2012, nurses at this Inspira Medical Center Woodbury clinic meet with referred patients to assess medical conditions, review medications, communicate any health concerns, verify follow-up cardiologist visits, and evaluate additional care needs. • Therapy Dog Program — As evidenced by research conducted at Inspira Medical Center Vineland, cardiac patients are four times less likely to refuse an opportunity to walk and walk longer distances at a stretch in the company of a therapy dog.
