6 M O R E T H A N 3 0 P E R C E N T of Americans want to use smart- phones or tablets for health m a n a gement or ser v ices, according to a 2013 report by the Offi ce of the National Coordinator for Health Information Technolog y. More than 60 percent want to communicate with providers electronically. But technology is only beginning to align fully with patient desires. Providers make use of remote devices such as Holter monitors to track the status of certain health conditions, but the information is not directly shared with patients. Currently, PGD is primarily collected via patient portals, which allow patients to exchange information with physicians electronically and, in some cases, respond to follow-up question- naires. More recently, mobile apps that capture patient data on a daily basis have become available, but few transmit the data to providers. At a recent Healthcare Information and Management Systems Society con- ference, Kristina Dudley Sheridan, MSc, Framework says that for effective data collection, e-health tools must inform, engage and empower patients. The right mobile IT can satisf y these require- ments, Sheridan says. The HealthAction Patient Toolkit, for example, will allow physicians to cus- tomize symptom checklists, details of treatment plans and additional e-health tools for each patient. Once the app is installed on a smartphone or tablet, patients will use it throughout the day to indicate the presence and severity of symptoms, record vital signs, and show compliance with treatment plans by marking the checklists provided. The app is set up to transmit PGD to providers regularly, and the physician version will track PGD in longitudinal trend lines. Making those patients and their caregivers more active partners with their clinicians could signifi cantly improve outcomes and lower costs, Sheridan says. As principal investigator for a new project — the Health Action Patient Toolkit — designed to more robustly capture and integrate PGD into the care setting, she emphasizes that to be useful for clinicians, PGD should be as complete as possible, customized for the patient's condition, and of suffi cient quality and quantity to indicate trends.
