In our cover feature, we profile WellSpan Health's estab- lishment of patient-centered medical homes (PCMHs) in its primary care practices. Using the PCMH model, providers, along with their care coordination team members, proactively partner with patients to help them achieve their health goals. The result is a better care delivery system, with the patient as the focus. WellSpan Urogynecology & Pelvic Reconstructive Surgery is profiled as well. Fellowship-trained specialists are able to accurately diagnose and effectively treat pelvic organ prolapse, urinary incontinence, recurrent urinary tract infections, overactive bladder, fecal incontinence and other conditions. We also learn about WellSpan Medical Group's Physician Dashboard, a data-driven approach to improving care for people who have or who are at risk of developing conditions such as diabetes, chronic pain, heart failure and ischemic vascular disease. Using information supplied by WellSpan's EHR system, physicians are able to study a problem thoroughly before attempting to solve it. In addition, we offer a special clinical section about oncology. We explore WellSpan's utilization of the NCCN Distress Thermometer to help identify and address oncology patients' emotional and psychological needs.
