NONPROFIT ORG. U.S. POSTAGE PAID CLAXTON-HEPBURN MEDICAL CENTER 214 King Street Ogdensburg, NY 13669 www.claxtonhepburn.org The Move to Better Patient Safety reduces the possibility of medication errors, and the use of bar coding increases accuracy and efficiency of caregivers. Caregivers will scan the bar code on the patient's wristband and on the medications to ensure the "five rights" of medication management: AT CLAXTON-HEPBURN, PATIENT safety is a top priority. In February 2012, a new service will help transform the care we provide. Bedside Medication Verification (BMV) allows caregivers to utilize bar code scanning technology prior to administering medications, confirming patient identity and medication information against the Electronic Medication Record. The new process • right patient • right medication • right dosage • right route • right time "Thanks to this advanced technology, we are able to double-check patient medication, which helps to prevent medication errors," says David Ferris, RN, MSHA, FACHE, chief nursing officer. "This initiative helps solidify our commitment to ensuring our patients receive the best, safest, and most advanced care." The BMV system utilizes important safety checks to ensure that patients receive the correct medications during their hospital stay. The process includes three main steps: 1. A hospital caregiver will scan the patient's ID bracelet and medication(s). Bedside computers will verify that the ID bracelet information matches the medications the physician has ordered for that patient. 2. The caregiver may provide the patient with information on new medications and side effects and answer any questions regarding the medications. 3. Once verified in the computer, the caregiver will administer medications to the patient. Our BMV process is another example of the importance that Claxton-Hepburn Medical Center places on patient safety and quality of care. We continue our commitment to bring the latest clinical advancements to our community, and we look forward to implementing this vital safety process throughout our system.
