This clinical collaboration establishes a new, innova- tive model of rehabilitation care in which a Montefiore geriatrician and physiatrist will work collaboratively on care decisions after a patient is discharged from the hospital in order to improve outcomes and provide optimal care in an outpatient setting. The sub-acute floor at the Hebrew Home, staffed by Montefiore phy - sicians, opened Oct. 1 and will accept referrals from all acute hospitals, as appropriate. The Helen Hayes Hospital Pharmacy Depar tment was presented with two top awards for excellence in pharmacy ser vices at the A SHP Midyear Clinical Meeting on Dec. 6, 2016. Comprehensive Pharmacy Ser vices named the depar tment the 2016 Clinical Site of the Year and recipient of the 2016 Best Practices Award for the Nor theast/ Southern region. Francine Weber, HHH Director of Pharmacy, and Shaindy Gluck, Pharmacy Clinical Program Manager, accepted the award at the A SHP gathering of over 20,000 pharmacy professionals. The Clinical Site of the Year Award recognizes outstanding achievement and leadership in clinical pharmacy programs and outcomes, suppor ting all aspects of hospital operations through clinical pharmacy systems and processes. The Best Practices Award — which the HHH Pharmacy has received for several years — recognizes excellence in pharmacy ser vices, with recipients needing a score of 95 percent or higher on CPS' annual Operational Audit, measuring compliance in all areas of the medication management process. WESTMED Medical Group (WMG) announced its continued expansion into Connecticut with the signing on of Nor walk Medical Group. Nor walk Medical Group, with current offices at 40 Cross St. in Nor walk, Connecticut, will join WMG early in the second quar ter of 2017. The well-respected practice in Norwalk includes 19 board- certified physicians and nurse practitioners offering primary care and a wide variety of medical specialties. As a multispecialty medical practice, Nor walk Medical Group has provided medical care to Fair field and Westchester counties for over 50 years. It has been recognized for achieving the highest level of Patient-Centered Medical Home cer tification by the National Committee for Quality Assurance. After joining WESTMED in the second quar ter of 2017, the office number for Nor walk Medical Group will still be 203-845-4800. Calvar y Hospital and the National Association of Chevra Kadisha (NA SCK) joined forces again at the Hospital's Bronx campus to educate more than 30 community rabbis, rabbinic students and chaplains from across the greater New York area. The event focused on advanced directives in Halacha, pain management, delirium and palliative care, hydration and nutrition, and the hospital chaplain as a liaison to the community rabbi. Attendees learned key medical information needed to provide accurate information to people seeking advice for loved ones or themselves. Frontier Healthcare Holdings, a New York-based developer and manager of ambulator y surger y centers, announced it has par tnered with AMSC's ownership group to open a multispecialty ambulator y surger y center in the Bronx. The new facility, which received approval to open on Dec.±5, 2016, at 951 Brook Ave., will feature gastroenterology, or thopedic, pain management and various other specialties. The 20,000-square-foot, state-of-the-ar t facility includes five procedure rooms and is expected to ser ve more than 8,000 patients annually. The center is expected to include five to 10 physicians and employ more than 30 healthcare professionals and administrative personnel. The ownership team includes Louis Rose, MD; Abdo Balikcioglu, MD; Rober t Slingsby; and Joshua Schwar tz. Frontier Healthcare will also own a percentage of the center. The building, which had been vacant for a number of years, is expected to house a number of other medical practices as well. For Frontier, the center represents its largest to date, both in the number of specialties it offers and in its physical size. Frontier now manages 14 centers, 13 of which are in New York. ■ The Helen Hayes Hospital Pharmacy Department Calvary Hospital and NASCK joined together for the third time to host an educational event for more than 30 rabbis, rabbinic students and chaplains focused on medical Halacha at the end of life.
