FOUNDATION BOARD OF DIRECTORS The Unity Health Foundation could not do what we do without the commitment of the many different groups of volunteers that help us further our mission to raise funds that will help improve the quality of healthcare in the communities we serve. About Us The Unity Health Foundation is a non- profit organization that is committed to medical excellence, compassionate care and to improving the overall health of the communities we serve. The Foundation's role is to support Unity Health by raising funds for equipment upgrades, renovations and special projects that enhance patient care. Gifts to the Unity Health Foundation enable Unity Health to provide additional services and programs which would either not be available or would require additional charges to our patients. The goal of the Foundation is to develop a lasting partnership with the community in order to preserve, sustain and develop Unity Health for generations to come. Ways to Give There are many ways you can support the Unity Health Foundation. All of our donors play an important role by providing funds not otherwise available to Unity Health, making possible significant and exciting health and wellness improvements in our community. PILLARS OF HEALTH Pillars of Health members contribute $100 or more annually to the Unity Health Foundation. As a Pillars of Health member you will be invited to our annual donor recognition event as well as other events and programs offered by Unity Health Foundation. MONTHLY GIVING When you join Pillars of Health you have the option of making your gift a recurring monthly gift. Your monthly gift will support the Unity Health Foundation year- around. A monthly gift of $10, $20 or even $50 will go a long way in advancing Unity Health's ability to provide quality patient care. MOTHER'S DAY & FATHER'S DAY TRIBUTES Every year, through our Mother's and Father's Day tribute program, you have the opportunity to honor the special Mothers and Fathers in your life with a gift to the Unity Health Foundation. All honorary or memorial gifts of $25 or more are listed in a special newspaper ad that runs on Mother's and Father's Day. This is a wonderful way to say thank you to those who have had a positive influence on your life and at the same time make an impact on healthcare in our community. TREE OF LIFE The Tree of Life is a gift of love and honor permanently mounted on the first floor of Unity Health – White County Medical Center's main entrance. Through your tax-deductible gift to the Unity Health Foundation, you will have the opportunity to add a leaf or stone with a special inscription for a loved one or someone you wish to honor or show appreciation. TRIBUTE GIFTS Gifts can be made to honor someone special or remember someone who has passed. Families may also choose to memorialize someone who has passed with a donation 'in lieu of flowers' for the funeral service. SPIRIT OF UNITY The Spirit of Unity Award is a way for patients and families to say thank you to a Unity Health team member or department who made a difference in their experience. The award will be given to any associate or department when a gift is given in their name to the Unity Health Foundation by a patient or patient's family. PLANNED GIFTS You can invest in the future of Unity Health by joining the Unity Health Foundation Heritage Society, members have expressed their long-term commitment to help continue the mission of healthcare excellence through gifts by will, life insurance gifts, charitable gift annuities or beneficiary gifts to the Unity Health Foundation. Members receive an invitation to the Annual Heritage Society Reception and invitations to special Foundation events.
