"We have a fantastic joint replacement program at UMC, and utilizing the CJR model makes us that much better," said Dawn Rakiey, CJR coordinator at UMC. "We use well- rounded, cohesive collaboration between our physicians, therapists and nurses to make the model work." CJR provides target spending prices to hospitals for patients who receive hip or knee replacements and includes costs incurred for 90 days post-discharge. UMC has stayed well below these target prices by implementing new practices, such as same-day post-op physical therapy and a mandatory pre-op education class for patients considering surgery. e new practices have resulted in quicker recovery times for patients. "Our physicians used to keep patients here for four or five days after surgery," Rakiey said. "Now, they only keep them an average of twodays." orough preparation and follow-up methods inspired by CJR have also allowed UMC to lower its readmission rates by 4 percent when compared to a 2012 to 2014 baseline period.
