MINIMALLY INVASIVE KNEE AND HIP REPL ACEMENT At one time, knee surgery was a major procedure involving significant hospital stays. Today, however, NYP Lawrence oŠers minimally invasive partial replacements that can preserve two-thirds of the original joint, Dr. Geller says, along with all the knee ligaments. Preser ving the a nterior a nd posterior cruciate liga ments is especia lly impor ta nt to ma king the knee "feel like a knee," he says. Patients a re more likely to be able to ma ke dyna mic cutting and twisting movements with these ligaments in place. The abilit y to spa re both is a technolog ica l brea kthrough, a nd patients return home the sa me day as surger y. "This is life-a ltering," Dr. Geller says. "Partia l knee replace- ments work well for ma ny yea rs, a nd patients ca n go back to ma ny intensely strenuous activities. Simila rly, we now have the technolog y to do full knee replacements while keeping a ll of the native knee liga ments, a groundbrea king procedure ava ilable only at highly specia lized or thopedic facilities." Hip surger y is performed in a nterior, minima lly invasive fashion at N Y P Lawrence, with the accompa nying reduction in pa in a nd leng th of hospita l stay. Dr. Geller a lso performs hip a nd knee revision surgeries. These a re cha lleng ing, he says, but rewa rding for patients who have suf fered w ith a badly impla nted joint. "Some of my happiest patients a re those who have had a poorly done knee replacement revised," he says. " W hen you can turn an unstable, swollen joint into a sturdy knee, you give patients back the ability to pa r ticipate fully in the activities of life that they va lue." ACHILLES TENDON REPAIR A not her specia l i zed procedu re, Ach i l les tendon repa i r, involves restoring function a f ter the tendon ha s r uptured. Typica lly an outpatient procedure, the repair involves locating a nd rejoining the ends of the Achilles tendon. The surger y of ten may be performed via minima lly invasive techniques, Dr. Greisberg notes. "Achilles tendon repa ir is one of the procedures I perform most frequently," he says. "This is a highly successful surgery." NON-RECONSTRUCTIVE ACL REPAIR Given the frequency of ACL injuries, ACL reconstr uction ha s lon g been i n h i g h dema nd . However, su rgeon s w it h Columbia Doctors a re now able to per form repa ir w ithout reconstruction, a ra re but effective a r throscopic procedure, Dr. Za mbetti says. W hereas reconstruction uses a gra f t from the patient's body or a donor, in repair, the surgeon uses a series of sutures and attachment devices to reconnect the ligament. A similar operation was performed in the 1980s, Dr. Zambetti adds, but it involved open surger y a nd leng thy rehabilitation. " Now, the instr umentation ha s been ref ined to ma ke it more accurate, a nd we have the necessa r y experience with these instr uments," he says.
