Reliant Home Health Offers Simple Solution for New Face-to-Face Documentation Requirement Reliant Home Health, a leader in home health care services in Central and North Texas, has devel- oped a simple, sticker solution to address the new requirement surrounding the Face to Face (FTF) Documentation. FTF must occur no earlier than 90 days prior to the start of care or within 30 days after the start of care. Reliant Home Health, a Medicare- certified home health agency, has developed a "sticker" that includes the primary reason for home health services (diagnosis), the reason patients are homebound, a section for comments, and the signature of the physician who completes the form and date on which is was signed. "We recognize the amount of adminis- trative paperwork physicians are already burdened with, so we're pleased to show these stickers to area groups of hospital- ists and other physicians to assist them to meet the new requirements," explains Russ Krengel, President of Reliant Home Health in San Antonio. "We've already received positive feedback that the sticker has helped facilitate the process of meeting the F2F requirement. Specifi cally, doctors will place a com- pleted sticker on their current orders when discharging patients for home health services. This sticker is acceptable in lieu of receiving the CMS F2F form. "A streamlined, simple solution using a sticker is an efficient and easy way to document patient encounters," says Krengel.
