The face of expertise The FACe OF CAnCer CAre Amit Panwalkar, MD Fargo John Lee, MD Sioux Falls John Bollinger, MD Bemidji A : In many cases, an internal bleaching technique performed by an endodontist can offer excellent results, with about an 80–90 percent success rate over five years. However, if the tooth has a large filling and is discolored, a more optimal treatment, both in terms of structural strength and long-term aesthetics, would be to consider all-ceramic (porcelain) restorations. The aesthetics for all-ceramic restorations are superior because there is no metal substructure, which can lead to a gray line around the gumline. Fabrication of these restorations involves removal of the pre-existing filling and tooth structure for adequate thickness of porcelain. The ceramic restorations are then bonded to the remaining tooth with adhesive cement. The bonding directly to the tooth structure provides the structural strength. The 10-year success rate for bonded, all-ceramic restorations is about 96 percent.
