8 Summer 2014 Healthy Directions An Unusual Double Delivery W hen 31-year-old Anna Profera of Manalapan found out she was having twins, she started preparing for double baby duty. She had already delivered a son at CentraState in 2011 as a patient of the obstetrical practice of Drs. Norman Back, Mark Seigel and Steven Goldstein, so she knew what to expect. "I loved CentraState. It was extremely well-organized," Anna says. "The nurses in that department were amazing. I never had a second thought about delivering anywhere else. I knew it was a great experience from the first time." With the twins, Anna experienced a normal, healthy pregnancy, and a strong desire to deliver the babies naturally, but as her due date approached, an ultrasound revealed that one of the babies was breech. "The second baby flipped from breech at 36 weeks, to transverse [sideways] at 37 weeks, to vertex [head- down] the 38th week. With both babies in a favorable position, Anna, her husband Louis, and I decided to induce labor to try for a successful vaginal delivery," states Mark Seigel, MD, board-certified ob/gyn and on staff at CentraState. "There is an art to the delivery of the second twin [from either the vertex or breech position] that requires experience, confidence, and skills from a coordinated team of obstetricians, neonatologists, anesthesiologists, and nurses." In October, Anna was admitted to CentraState for a scheduled induction of labor; she had been contracting at home, which made the cervix more favorable for a successful induction. With meticulous monitoring of the twins (now both in the head-down position), Anna progressed to full dilatation and started to push. "We wheeled Anna into the operating room so that she could deliver the first twin vaginally. We routinely deliver twins in the operating room in case of an obstetrical emergency involving either or both of the twins, or the mother. The entire health care team is assembled in case of an emergency, or the need for an emergency cesarean section," Dr. Seigel states. Anna vaginally delivered the first twin, a girl named Arianna weighing almost 5 lb. 7.4 oz., at 12:21 p.m. The head of twin B was directed into the pelvis by Dr. Seigel. Artificial rupture of the membranes secured the position of twin B into the pelvis. Once the fetal monitoring reassured him that twin B was healthy, Dr. Seigel decided to transfer Anna back to the laboring room where she would be more comfortable and allow labor and pushing to be more natural. At 1:49 pm, twin B, a boy named Louie weighing 7 lb. 1.55 oz., made his debut via Anna's second vaginal delivery of the day—a full 88 minutes after the birth of his older sister. "I couldn't have asked for a better team," Anna says. "The next morning after delivery, I even had the same nurse that cared for us when I had my first child." Despite the unusual nature of Anna's delivery, both twins were healthy and are doing great. "It was an amazing experience," Anna says. "Everything turned out naturally. I was so comfortable. It was like I was at home. The experience for me was even easier than the first time. It was so good I wouldn't hesitate to go to CentraState again."
