Case Study A 51 year old female sustained a head-on motor vehicle collision which resulted in a calcaneal fracture and tibiotalar contusion of her left foot. A year after her accident she presented to our office with extreme pain to her ankle and subsequently an MRI confirmed a posterior medial talar dome lesion. She underwent a medial malleolar osteotomy with an osteochondral autograft transplant to repair the lesion (figure 1). She did well for a year after surgery but then later presented with continued pain and swelling of her ankle with locking up of the ankle joint and some clicking when walking. Her pain progressed to the point where she constantly needed pain medication and her work was suffering. Bracing and other conservative methods were exhausted and she decided to have total ankle replacement. Ankle fusion was offered to the patient, but she did not want to have an ankle that did not move. Three years after her accident she underwent a total ankle replacement with the Wright Medical Inbone 2 recon- structive type of implant (figure 2). Three months later, she has progressed well, pain is significantly better, and she is walking and back to work. Currently, ibuprofen is keeping her pain controlled throughout the day. She has been pleased with her decision and is able to get back to riding her motorcycle.
