Our cover story features the Accountable Care Coalition of Syracuse. The ACC of Syracuse is a partnership between Universal American Corporation, its subsidiary Collaborative Health Systems (CHS) and Family Care Medical Group PC — a growing network of local primary care physicians. The ACC of Syracuse establishes a more integrated continuum of care, as providers throughout the ACO have access to patient medical records through HealtheConnections, the Regional Health Information Organization of Central New York. Taking a collaborative approach to improving the region's health has the ACC on the right road. Our first interior feature is focused on the Syracuse Orthopedic Specialists' Cartilage Center of New York. Experts at SOS provide innovative treatments to treat cartilage injuries in younger and more active patients. Cartilage restoration procedures are not widely available, but are revolutionizing treatments in the field. Our second feature is on the Heart Group of Syracuse.
