n 2014, CHI St. Joseph's Health received a three-year,
$597,693 grant from PrimeWest Health's Community
Reinvestment Grant Program. With this financial backing,
CHI St. Joseph's Health Weight Management Center has
developed a new Lifestyle Navigator Program to help
guide patients through weight loss.
"Our vision is to help community members better navigate
the broad range of lifestyle management services that we
offer," says Leah Walters, RDN, CDE, CHI St. Joseph's Weight
Management Center manager. "Many people don't realize
how expansive our services are, or they don't know how to
access them. Our Lifestyle Navigator Program cuts through
the confusion."
NAVIGATING THE SYSTEM
With the help of Brett Fredrickson, an exercise
physiologist and the new Lifestyle Coach and Navigator
at the CHI St. Joseph's Weight Management Center,
you can now easily access the services that best fit your
needs. Either you or your healthcare provider can contact
Fredrickson to set up a meeting. If you're visiting with
a local provider at Essentia Health, the Lifestyle Coach
and Navigator can meet with you and your provider
right away. During the meeting, you'll discuss your basic
medical history, current health and weight status, and
personal goals. No referral is needed—anyone can reach
out to the center.
"I ask patients what they've tried in the past—what has
worked and what hasn't," Fredrickson says. "I lay out all the
options in terms of the programs and services offered here,
and then I help them get plugged in."
A new grant to enhance weight management
services at CHI St. Joseph's Health means you
can find the help you need to lose weight
without getting lost in the healthcare maze.
