QUALITY For patients with diabetes who are admitted to Syosset Hospital, which is part of Northwell Health, close monitoring of their condition can mean the difference between a full recovery and continuing challenges. Four areas of inpatient data were examined for The Joint Commission Advanced Inpatient Management of Diabetes recognition: ➜ HYPOGLYCEMIA — We decreased the incidence of hypoglycemia to only 8 percent of patients with blood sugar measurement of less than 70 during hospitalization. ➜ HYPERGLYCEMIA — We decreased the incidence of hyperglycemia to 20 percent of patients who had blood sugar measurement greater than 250 after the first 24 hours of hospitalization. ➜ A1C — We increased the percentage of patients who had an A1C test (measuring blood sugar levels over several months) performed within the last three months to 94.6 percent. ➜ EDUCATION — We increased the percentage of patients who were provided diabetes education prior to discharge to 91.7 percent. "Achieving this certification demonstrates that we provide quality care and best practices and are proactive instead of reactive when it comes to diabetes management," said Meg Diliberto, ANP-BC, CDE, coordinator of diabetes inpatient management and certified diabetes educator with Syosset Hospital. "We take a multidisciplinary approach that incorporates an individualized plan of care based on the needs of patients. We also pride ourselves on continuity of care. Once patients are released from the hospital, we refer them to our Diabetes Wellness Outpatient Program where they can learn to improve diabetes self-management skills through education and support." BONES AND JOINTS Data analysis for total hip and knee replacements and for spine surgery focused on several areas, including length of stay, pain management and how soon patients walked after surgery. "This certification sets us apart as a center of excellence," said Debra Schulman, RN, MBA, CCRC, senior administrative director of orthopedic surgery at Syosset Hospital. "We received the gold seal of approval from The Joint Commission by demonstrating patient safety and quality for our total hip replacement, total knee replacement and spine surgery programs." By the Numbers Syosset Hospital performed more than 1,000 TOTAL JOINT REPLACEMENTS IN 2015. For patients who had hip or knee replacement or spinal surgery, only 0.3% had blood clots compared to a national average of 2.4 percent. In November 2015, 94% OF HIP REPLACEMENT PATIENTS AND 96% OF KNEE REPLACEMENT PATIENTS STAYED WITHIN PATIENT-DEFINED ACCEPTABLE PAIN LEVELS. 100% of patients admitted to Syosset Hospital with diabetes had an A1C test performed.
