Healing doesn't stop when you leave the hospital. Sometimes your recovery needs help. Safety First Finding quality care you can trust often means taking a look at a team's safety record. The Centers for Medicare & Medicaid Services (CMS) rates nursing homes on the quality of care they provide, which allows consumers to make informed decisions when choosing among organizations. The highest rating possible is five stars. Similarly, The Joint Commission, a not-for-profit organization, evaluates health organizations for following established protocols and offers their seal of approval through accreditation. Both CMS and The Joint Commission have reviewed our processes and awarded top ratings to CHI St. Joseph Health Skilled Nursing and Rehabilitation. Locally, we've also been recognized as the best skilled nursing center in the Brazos Valley two years in a row by "The Best of the Brazos." Helping You Heal—and Get Back Home During recovery from an injury or illness, going home after the hospital may not be the safest option. At CHI St. Joseph Health, we understand that every person's wellness journey is unique. That's why we oer transitional care, an option that provides therapy and medical services for those who may need to extend their healthcarestay. "Our nurses, therapists and speech pathologists partner with our social worker and dietary manager to oer each resident an individualized care plan," said Anthony Ughetti, executive director at CHI St. Joseph Health Skilled Nursing and Rehabilitation in Bryan, Texas. "The team identies guests' individual care needs, and then they provide services to help them reach their highest level of indepenence and freedom." When creating care plans, we honor personal preferences, taking food likes and dislikes to heart and allowing for exibility in wake and sleep times, Anthony said. We modify therapy schedules as well to make time here feel as it would at home—relaxed and comfortable. Residents also have the option of attending social events coordinated by our activities director. WHY WE EXIST "Our focus is on transitional care—our guests want to return home as quickly as they can," Anthony said. "Our therapy and interdisciplinary team provide the right mix of nurturing and encouragement to allow people to return home safely and quickly." On average, most residents spend 26 days in transitional care before they are ready for their next step, which may include going home and receiving additional support from home health services. CHI St. Joseph Health Skilled Nursing and Rehabilitation provides therapy services six to seven days per week in several locations, depending on individual needs. Most residential centers oer only ve days per week. "We are a community resource," Anthony said. "We can guide people back to health both during and after their stay here. We help people get better and get back home faster. If someone needs assistance, they don't need to go it alone—we can help."
