Now, as a CHI Franciscan Health patient, you can request re lls anytime, anywhere, using your iPhone® or Android™ phone. Install the free mobileRx® Pharmacy app on your smartphone and enter your pharmacy's phone number (10-digit, area code rst, no spaces or dashes). Express Re lls lets you enter up to three prescription re lls at a time. Repeat if you have more prescriptions to re ll. You will be noti ed immediately if prescriptions are accepted or if there is an error in the re ll request. Scan a Prescription with the barcode label on your prescription bottle and click Submit Re ll(s) to request a re ll. Pharmacy Information provides quick access to your pharmacy's address, hours and phone number, and lets you change your chosen pharmacy. Active Rx and Rx History screens put your vital prescription information right at your ngertips. Within your Patient Pro le, Active Rx lists the prescriptions you have available for re ll at any time, while Rx History lists prescription details, including dosage and the date of your last re ll. Did you know you can get your prescriptions mailed to your home using our mobileRx app? Just log in, select your delivery method and have them sent to your home. Thanks to MyChart, you can access your medical records at any time, anywhere summer adventures lead. With a few clicks, you can view medical information, including immunizations, blood pressure readings, lab records and more. You can also access electronic medical records 24/7 and reach out to your primary care provider when and where it's convenient for you— using your computer, tablet or smartphone. Patients with a Franciscan or Harrison HealthPartners primary care provider can set up a MyChart account. With MyChart, you can view your medical chart, review your current medications, securely send medical questions to your primary care provider, request prescription refills from your primary care provider, view normal test results (your provider will call about abnormal results), check scheduled appointments and pay bills. MyChart makes it simple to streamline health care for your entire household. If you have children under the age of 18, you can access their health records to more easily manage immunizations and schedule medical appointments. If you care for an aging parent, you can access their records with permission. Please note, if you are hospitalized or treated at St. Elizabeth Hospital, your patient records will be stored in another electronic portal called My Record. Your My Record account will allow you to view a summary of your hospital visit, hospital lab and radiology results, test results and discharge instructions. St. Elizabeth plans to move to the MyChart platform in 2016. In order for you to set up your MyChart account, you will need to get a password from your primary care clinic. You can also download the MyChart app for your Apple or Android phone and access your account from your phone any time you need to.
