CHI Friendship Person-Centered Excellence Accreditation 2015 Progress CHI Friendship received our four year accreditation in March of 2015 and identied three areas to improve our services. Below are the three areas and all of the accomplishments made within each action step throughout the past year. By creating tools, supports, and processes, people will have increased self-determination and choice making in the areas of their personal nances, the development of their personalized plans and their community connections. PEOPLE HAVE INCREASED COMMUNITY CONNECTIONS Complete One Community Connection Training for Each of the Following Areas: DSPs, Parents/Guardians, and Interdisciplinary Teams A meeting was held with selected Supervisors to discuss training materials and expectations for training on making community connections. PowerPoint created to use for training. Trainings on community connections were conducted in November 2015 at the General Sta meetings in both Fargo and Grafton. Host Six Community Education Classes (three in Fargo and three inGrafton) This committee met several times to discuss ideas for community classes, presenters, places to hold the classes, and various types of advertisements. A person supported, who currently teaches Zumba® classes twice a month at Courts Plus, held a community class in Fargo at Hope Lutheran Church on September 24, 2015 and at Collette Fitness Center in Grafton on September 25, 2015. Christmas Decorating with Heidi held on December 7, 2015 in Fargo. The class made a holiday decoration and several holidaytreats. A Lefse making class was held in January 2016 inGrafton. Currently exploring possible health classes and presenters. Explore Agency Signature Event Options An agency-wide SComm was sent to request ideas for signatureevents. A decision was made to hold separate signature events in Grafton and Fargo. Surveys were developed for sta to vote on various signature events. Committee received 111 responses back. A decision was made to have our signature event in Grafton on the weekend of Summerfest with a carnival-type event. Identify and Implement Two Mechanisms so DSPs Can Have More Knowledge of Community Events The committee discussed several mechanisms, such as monthly community calendars, activity binders, automated message systems, Therap, Facebook and activity boards. Activity binders are currently being used in Fargo and will now be implemented in Grafton. Explored various phone vendors for automated message systems. After reviewing cost and other factors, it was determined that this is not a feasible option at this time. Developed a "Community Connections Corner" for both 554 and 801. A display case has been set up in both locations containing brochures, magazines, iers, print os, etc. advertising community events.
