Welcome, SPECIALISTS An Ideal Plan for Discharge NOT HAVING THE inpatient hospital space to accommodate patients is a trend that has not been uncommon at UMC Health System. It is also a pattern that the hospital seeks to address with the IDEAL Discharge initiative. "One of the biggest challenges we have faced is having to board patients in operating rooms or the Emergency Center while waiting for inpatient rooms to become available," said Lisa Barrington, M.S.N., R.N., Director of Operational Excellence at UMC Health System. "To minimize this issue, we are asking our physicians to use an order called 'patient discharge anticipated within 24 hours' when appropriate." When a physician anticipates that a patient is almost ready to go home, he or she will signal to the care team to begin PLANNING AHEAD IS KEY TO MAXIMIZING EFFICIENCY, IMPROVING COORDINATION OF CARE AND ENHANCING THE PATIENT EXPERIENCE. preparations to make timely discharge possible. Specifically, the care team will focus on three key components: + Improving patient flow by focusing on planning ahead + Enhancing transitions of care by having physicians provide discharge summaries for each patient within 72 hours of discharge + Optimizing medication management by ensuring that patients are comfortable with new routines and that all medications are safe for each individual patient "ese concepts intuitively make sense to our physicians and will tremendously benefit our patients by reducing risk of complications," Barrington said.
