www.SJAHS.org | CHI St. Joseph's Health 7 "We are proud to lead the way in reducing healthcare costs and helping people 'Experience better health.' This grant allows us to help create a healthier, happier community. We'll work with other providers and organizations so that all patients, including our underserved populations, can achieve their weight management goals." BEN KOPPELMAN, PRESIDENT CHI ST. JOSEPH'S HEALTH Can't Get Here? The Community Reinvestment Grant allows CHI St. Joseph's Health to assist patients with medical transportation needs. To reduce barriers, the Lifestyle Coach and Navigator will work with local transportation services to help you make your medical appointments at no extra cost. Sounds great! See you then. Hi, Kelsey! Absolutely. Let's sit down this week and talk about your health goals. We can come up with a plan together. When are you free? Hi, I'm Kelsey. I found out about your Lifestyle Navigator Program online. I'm 17 and unhappy with my weight. Can you help? Awesome. My mom and I can come in tomorrow at 11. Kelsey: Kelsey: Brett: Brett: Help for Youth CHI St. Joseph's Health Weight Management Center is expanding to include services for children and adolescents ages 6 to 18. Childhood obesity is a significant problem in Minnesota, with 14 percent of children ages 10 to 17 considered obese. If your child is struggling with unhealthy excess weight, help him or her to take action before it becomes a lifelong problem. Both youth and adults can call or text Lifestyle Coach and Navigator Brett Fredrickson directly at 218.255.7527. When texting, provide your name, how you heard about the Lifestyle Navigator Program, and any questions you have. YOUR FIRST STOP FOR BETTER HEALTH Services available at the CHI St. Joseph's Weight Management Center include nutrition education and counseling, exercise programs, behavioral health, and tailored lifestyle management. The Lifestyle Navigator Program also bridges the gap between other resources in the community. CHI St. Joseph's Health works collaboratively with a number of local healthcare providers, local transportation programs, and agencies that offer physical and behavioral health services and reduce barriers to making healthy changes. By using the Lifestyle Navigator Program as a stepping-off point, you can get connected to these other community services with ease. "My passion is helping people achieve their health goals," Fredrickson says. "As the Lifestyle Coach and Navigator, I ensure patients have everything they need for success and help them work through any problems along the way." MORE THAN MEETS THE EYE Because of the Community Reinvestment Grant, CHI St. Joseph's Health will be able to continue offering preventive and lifestyle-focused services that help the community stay healthy and keep healthcare costs down. "We may be a small-town critical access hospital, but we've always tried to provide more than just basic medical services," Walters says. "With this new grant, we've made it easier than ever to get help losing weight."
