Annual Gala October 24 Highline Medical Center Foundation partners with donors and the medical center to create a new program to help the most vulnerable in our community. FOUNDATION NEWS SAVE THE DATE! HIGHLINE MEDICAL CENTER FOUNDATION is excited to partner with the medical center on an innovative new program to help members of our community who are unprepared to face health issues. Modeled after several national programs, including a project in Camden, New Jersey, and other Catholic Health Initiative programs in Kentucky, our program began to see its first participants in January 2015. This pilot project, which reaches an initial group of 100 patients and will expand to involve 200 by year three of the program, will enable us to provide health care to the most ill and vulnerable in our community. Managed by Highline's Home Health department, staff will take a proactive approach and create a multifaceted program comprising registered nurses, social workers and community navigators, all of whom will work as an integrated team to engage and support patients in meeting a wide range of needs that impact their health, such as the provision of prescriptions, housing and meals, access to community services, and self-care. The vast majority of the individuals in this program have multiple chronic illnesses with little or no support system or resources to meet their needs. This is a cooperative project to be funded by angel investors and donors, as well as other private philanthropy sources and grants, and it will ultimately reduce medical center expenses. We are excited to be a part of this critical initiative to serve our community.
