VNA HOMECARE AND ITS FIVE AFFILIATED ENTITIES ARE NOW NA SCENTIA HE ALTH ¢ A UNIFIED HE ALTHCARE SYSTEM BUILDING ON THE RICH HISTORY OF ITS PREDECESSORS BY PROVIDING HOLISTIC, INžHOME CARE TO A L ARGER ARE A OF CENTR AL AND UPSTATE NE W YORK. W I T H R O O T S T H AT stretch back more than 125 years, to the 1890 founding of the Visiting Nurse Association of Central New York, Nascentia Health reflects what VNA Homecare, VNA Homecare Options LLC, Home Aides of Central New York Inc. and their partner organizations have become: a healthcare system that is greater than the sum of its parts. "By becoming Nascentia Health, we are redefining, rather than reinventing, ourselves," says Andrea Lazarek-LaQuay, RN, MS, Chief Clinical O'cer at Nascentia Health. "We started as separate organizations, providing isolated silos of care. As we grew and changed, we did so together, and those barriers came down. We needed an identity that reflected the new us: a single healthcare system devoted to caring for a variety of patients in their homes that has much more to o„er the individuals in our care and their families." SERVING MORE OF NEW YORK One of the key elements of the transformation of VNA Homecare into Na scentia Hea lth is the ex pa nded ser v ice footprint of 48 counties — a ll of centra l and upstate New York — for VNA Homecare Options, a managed long-term care plan for Medicaid-eligible individua ls. Through partnerships with primary care providers in both regions, VNA Homecare Options a llows patients to receive care from the clinicians they are familiar with — provided they are part of the network — in the environment in which they are most comfortable: home. "The legacy of the organizations we come from is one of serving patients in their homes and helping them be as healthy and independent as possible," Lazarek-LaQuay says. " With VNA Homecare Options, we do that even more e„ectively. For patients, getting the care they need is e'cient and streamlined. We work with their providers to create and coordinate a plan that features all of the services they need, including in-home care, transportation to and from medical appointments, prescription refills, imaging tests, and outpatient services." COMING SOON: A BIGGER, BETTER HEADQUARTERS Nascentia Health's organizational reinvention necessitates an infrastructure to match. That is why, in early 2018, the healthcare system will vacate its nearly century-old facility at 1050 W. A New Beginning for a LEGACY ORGANIZATION BY THOMAS CROCKER 1 4S P E C I A L F E A T U R E
