" Every patient in our care has a unique story, and no one knows it better than their physicians. We partner with community-based physicians and other providers to create a single, unified care team. One size does not fit all when it comes to the home care population. We work with physicians to create innovative solutions to fit each patient's needs." ANDREA LAZAREK•LAQUAY, RN, MS, CHIEF CLINICAL OFFICER AT NASCENTIA HEALTH Genesee St. in Syracuse and move into their new headquarters, a multimillion-dollar, 47,000-square-foot space built on the same property. Once the transition is complete, the old building will be demolished. "Our current home isn't meeting our needs as a growing organiza- tion," Lazarek-LaQuay says. "We are expanding our services as well as our number of employees. The new building is designed to support a mobile workforce that includes both o'ce- and home- based team members. The majority of our nearly 550 employees will be based in the building, and it will serve as a landing pad for those who are remote. The floor plan features open spaces where sta„ can collaborate." Employees will not be the only ones to benefit from Nascentia Health's new headquarters. Participants in the Adult Social Day Program — which provides daytime social and therapeutic support for individuals with long-term care needs — will enjoy a larger, 4,300-square-foot space with more bathrooms and showers, as well as a courtyard. The larger facility will allow the program to grow and accommodate up to 45 participants a day. "The courtyard will be a lovely place for our Adult Social Day Program participants to sit on a bench and enjoy the outdoors," Lazarek-LaQuay says. "The program's new area will allow us to o„er more activities and exercises in a visually appealing space." Patients will also benefit from an expanded Continuing Care Call Center where they can receive referrals to any home- or community-based service, regardless of whether it is part of Nascentia Health. A FUTURE SHAPED BY COLLABORATION Partnership with patients and their families, as well as clinicians throughout the state, is a hallmark of Nascentia Health, and it's poised to play an even larger role in the organization's future. Lazarek-LaQuay envisions collaborating with emergency medical technicians to improve care for patients in their homes, creating a Hospital at Home Program that would spare patients trips to the hospital for certain services. In addition, she plans to work with physicians to revive a largely bygone practice: house calls. "All of these e„orts will be important as we seek to keep patients out of the hospital when appropriate," Lazarek-LaQuay says. "We are building a healthcare system without walls."
