DISCOVERY, SELFDETERMINATION, AND CHOICE Personal Outcome Measures (POM) are a key component of CHI Friendship's annual assessments in preparation for creating a person's annual Person-Centered Support Plan. POMs are standard evaluations that we incorporate into our services for each person we support while working with our accrediting body, The Council on Quality and Leadership (CQL). POMs are applied to each person on an annual basis. CHI Friendship began collecting formal data in 2006. In 2014, the POM data set consisted of 129 interviews. The total POM data set includes 348 interviews since 2006. 2014 HIGHLIGHTS Our Social Capital (connecting to the community) Index shows an increase of Bridging and Bonding outcomes from 79 to 81 percent. Our Basic Assurances Index for outcomes shows an increase from 89 to 90 percent. Our Basic Assurance Index–Health and Safety shows an increase from 87 to 89 percent. Several other outcomes and supports remain at a high score: Friends supports increased from 78 to 92 percent. Social Roles outcome increased from 49 to 52 percent. Social Roles supports increased from 75 to 81 percent. People Choose Services outcome was 98 percent. This is a reection that people are living self-directed lives. The People Are Respected outcome was 99 percent. BUILDING COMMUNITY LIFE Each person is supported to have social roles and friends. Our aggregate data shows that we have made progress in this area over the last year. Our Social Roles outcome increased from 49 to 52 percent and supports from 75 to 81 percent. Our Friends outcomes increased from 68 to 70 percent and supports from 89 to 92 percent. THE RESULTS ARE IN! Each year, our Satisfaction Survey offers the people we support, their parents, and their guardians a chance to provide feedback on our services. In 2015, we surveyed 186 people, who awarded us standout ratings in areas such as feeling safe, being welcomed by CHI Friendship team members, and having access to transportation. CHI Friendship received a final rating of very good or higher on all survey questions, for an overall score of 4.35 out of 5, and the survey provided us with insights into how we can better support our clients in the months and years to come. "These results say a lot about the dedication of our staff and their commitment to our core values of reverence, integrity, compassion, and excellence," said Jeff Pederson, President of CHI Friendship. "We are always working to improve our services. The input of those we support and their loved ones plays a large part in our success." WINNERS AMONG US CHI Friendship is extremely proud of our employees and the people we support who have been recognized as leaders in their communities. AWARD WINNERS The Association for Persons in Supported Employment (APSE) recently recognized the "Best of the Best." Several people who CHI Friendship supports were recognized. We congratulated Aaron Burns, Jamie Woodbury, and Adam Hammer for their hard work and dedication. Kimberly Riggs was recognized by APSE for her entrepreneurship, and Jay Sorum was honored by the Fargo Metro Area Mayor's Committee as Entrepreneur of the Year for his business, Jay's Candy Creations. TOP PROFESSIONAL AWARD CHI Friendship is extremely proud of Steve Ouren, aDirect Support Professional with CHI Friendship. The North Dakota Association of Community Providers named him a 2015 Direct Support Professional of the Year. Steve is recognized as an outstanding sta member who shows commitment, compassion, and expertise in the workplace.
